Payment

Payment is “fee for service” and due in full at the time the services are provided. Cash and checks are accepted in-person. Credit card payments are accepted via Paypal in advance of the service. Please request an invoice to pay by credit card. Services provided at the Spring Valley office will be paid to Comprehensive Cardiac Care. Credit card payments can be made onsite at this location only.

Insurance
At this time, Elite Nutrition does not accept insurance. You will receive a receipt and superbill you can use to submit to your insurance company to request reimbursement if your service qualifies. In general, sports nutrition therapy and preventative nutrition therapy are not covered by most insurance companies. However, weight management, cardiovascular nutrition, and other medical nutrition therapy may qualify for coverage. In addition, if you have diabetes or another nutrition-related condition like Celiac’s disease or Crohn’s disease, your services may be covered.

You should contact your insurance company before making an appointment if you need reimbursement. Your insurance company should tell you if a doctor’s referral is required and which services are reimbursable. I am an “out of network” provider for all insurance companies.

Cancellations
At least 24-hours notice is required for all cancellations, except for emergencies. You will be charged for 50% of the service fee for any non-emergency cancellation less than 24-hours from your scheduled appointment. Please contact Elite Nutrition at your earliest opportunity to cancel your session and reschedule at a later date.

Cancellations without notice will be charged 100% of the service fee.
